Roadmap: Complete Eligibility Paperwork 

Timeline: ~1 day to 2+ weeks

Once your Medicaid application is being processed (it does not have to be approved yet), the next step is to gather and fill out the paperwork that the waiver program needs. This packet helps show what supports you or your family member needs.

You will complete these forms:

If you or your family member has an Intellectual and Developmental Disability (IDD), include the paperwork that shows IQ and/or adaptive skills scores. Make sure the report includes the scores and the evaluator’s signature.

The Long-Term Care Professional Medicaid Information Page (PMIP) must be filled out by your family member’s primary care doctor. It also needs to be signed and dated within 90 days before you turn in your packet.

When everything is ready, send your packet to your local Case Management Agency (CMA). A CMA is the office that reviews your paperwork and helps guide you through the waiver process. If you have questions or get stuck, you can call your local CMA—they can help you understand what is needed and help you with the paperwork.
