Roadmap: Case Management Agency (CMA) Intake

Timeline: Up to 90 days

After you turn in your packet, your Case Management Agency (CMA) will check your paperwork. They will contact you (by phone, email, or mail) to let you know what happens next. It is normal for them to ask questions, double-check information, or tell you if something is missing and needs to be added.

If your packet can move forward, the CMA will assign you a Case Manager. Your Case Manager will reach out to set up the 100.2 assessment, which is the next meeting where you talk about what help you or your family member needs day to day
