Roadmap: Service Planning & Choosing Your Supports
Timeline: ~1–3 weeks after eligibility is approved (can vary)
Once you or your family member is approved, your Case Manager will work with you to make a service plan. A service plan is a written plan that explains what help is needed, what goals you have, and what services will be used. You will talk about your daily routine, what is going well, and where extra support would make life safer and easier for you or your family member.

There are different ways services can be provided. One way is to work with an agency, including In-Home Support Services (IHSS). With an agency, the agency helps with hiring workers, training, scheduling, and paying workers. Another way is Consumer Directed Attendant Support Services (CDASS). With CDASS, you or your family member has more control—you choose and manage workers yourselves, and you follow an approved budget.
You or your family member get to make choices about providers. Your Case Manager will explain the options and help you understand them. Providers are not all the same, so it is okay to ask questions and take time to decide (for example: who will be coming to your home, what times they can work, and what help they can provide). If you or a family member will be a paid caregiver, that is a real job—make sure the plan works for you and for your family.
