Roadmap: Starting Services 

Timeline: ~1–4+ weeks (depending on provider and staffing)

Once you or your family member chooses the preferred type of care:

  • Consumer Directed Attendant Support Services (CDASS)
  • In-Home Support Services (IHSS)
  • An agency

The next step is onboarding (getting set up to start). The provider (CDASS / IHSS) or agency will help you fill out start-up forms, review service details, and make a plan for the schedule. Then services can begin once staffing and dates are confirmed.

  1. Apply for Long-Term Care Medicaid
  2. Identify Disability Type
  3. Complete Eligibility Paperwork 
  4. Case Management Agency (CMA) Intake
  5. The 100.2 Assessment
  6. Service Planning & Choosing Your Supports 
  7. Starting Services
